If you attended the 2025 PACHC Annual Conference and Clinical Summit last week, please remember additional action is needed to claim your continuing education credits. To claim your credits, follow these instructions:
- Create an account on the Weitzman Education Platform if you do not have one already. If you claimed credits last year, you already have a login. Reference How do I create an account? if needed.
- Visit the 2025 PACHC Annual Conference & Clinical Summit event site.
- Select the Register button to register for all conference sessions, regardless of your attendance.
- Then, in the table, select the links for each breakout session you attended.
- Within each activity, select the Continue tab to complete the session evaluation. You will need to complete a session evaluation for each breakout session you would like to claim credit for attending.
- After completing session evaluations and claiming credit for each session you attended, you can download an overall conference certificate appropriate to your credentials.

You must claim credit and download your certificate for the activity by April 25, 2026. For compliance reasons, the Weitzman Institute cannot modify this deadline.
