PACHC is excited to continue partnering with the Commonwealth of Pennsylvania, Department of Health, Bureau of Family Health (BFH) to increase access to services for Pennsylvania children and youth with special healthcare needs (CYSHCN) and their families. The BFH administers the Title V Maternal and Child Services Block Grant (MCHSBG) for Pennsylvania, which is funding this partnership. The purpose of this grant is to provide access to quality health care for CYSHCN and families with low incomes and/or limited availability of care. Through the implementation of care coordination methodologies, the project will enhance access to evidence-based, team-oriented, and whole-person health care for clients and their families. Care coordination involves the deliberate organization of patient care activities and the effective sharing of information among all individuals involved in a patient’s care to ensure safer, more effective outcomes. These initiatives will address, but are not limited, to: coordination among specialized healthcare providers; the reduction of financial barriers faced by low-income and benefit-ineligible families; the mitigation of language barriers and lack of translation services; and the provision of organized, accessible care for families.
