Nominations are open through April 11 for eight awards that will be presented at the 2025 Community Health Institute (CHI) and EXPO in Chicago, August 17-19. Self-nominations are accepted. Nominations require contact information of two supporting nominators. The various awards recognize overall excellence and specific contributions to clinical practice, health center development, migrant/farmworker health, and education and training. There are also specific awards for a senior financial professional and a Primary Care Association staff member. You can also nominate someone with at least 25 years of service for a Lifetime Achievement Award. Additional details are provided in the online nomination form. Nominators and nominees must belong to a NACHC Organizational Member community health center, a chartered affiliate Primary Care Association, or Health Center Controlled Network member of NACHC.
