PA Navigate Training for Health Center Clinicians Webinar
Join PACHC and PA Navigate on Jan. 21 from 1:00 – 2:15 pm for a webinar titled PA Navigate Training for Health Center Clinicians. Presenters will address an overview and benefits of PA Navigate, the new PA statewide resource and referral tool, discuss grant funding opportunities, and demonstrate the use of PA Navigate by providers and community-based organizations. About the Speakers:
Carly M. Pasinski, RN, MSN
Carly Pasinski is a Systems Analyst at ClincialConnect Health Information Exchange, focused on creating and implementing PA Navigate.
PA Navigate is a new statewide resource and referral tool designed to support Pennsylvanians with social needs by connecting them to community organizations ready to help. ClinicalConnectHIE recognizes the impact that social determinants of health have on an individual’s well-being, and, through PA Navigate, is striving to make social data as sharable amongst providers as clinical data is.
In her ten years of neonatal and pediatric nursing and care management, Carly Pasinski saw firsthand the effects that social drivers can have on overall health and wellness. She has also felt the pain of finding appropriate resources to address those needs. She brings a unique perspective to the project and has a passion for creating the absolute best product for Pennsylvanians.
Carly has earned a Bachelor of Arts in Speech and Hearing Science from The Ohio State University, a Bachelor of Science in Nursing from Duquesne University, and a Master of Science in Nursing from Capella University. She currently resides in Pittsburgh, Pennsylvania.
Laura A. Mosesso
Laura A. Mosesso is the Compliance and Outreach Manager for ClinicalConnect Health Information Exchange (HIE). Ms. Mosesso is responsible for ClinicalConnect HIE’s business development, compliance, data quality, health equity, and marketing activities.
ClinicalConnect HIE is western Pennsylvania’s health information exchange which securely delivers clinical and social care data for better quality patient care, better care coordination, monitoring, and reporting to hospitals, physician practices, skilled nursing, insurers, federally qualified health centers, and social service agencies, located throughout the Commonwealth. ClinicalConnect HIE is honored to be one of the four PA health information exchanges selected by the PA Department of Health and Human Services (PA DHS) to implement PA Navigate. PA Navigate is the new statewide community information network designed to address health and social care needs for Pennsylvanians by connecting them to community services.
Ms. Mosesso is an enthusiastic, people-oriented professional with more than 30 years of expertise in account/client management, health information management (HIM), information systems, product development, project management, regulatory compliance, and sales and marketing. In addition to her experience in healthcare and health information exchange businesses, Ms. Mosesso has worked in the health insurance industry, with healthcare software, electronic health record and consulting companies, and in medical simulation settings. Ms. Mosesso has a Bachelor of Science Degree in Health Records Administration from the University of Pittsburgh.
