Lowering the Pressure: Achieving Hypertension Control at the Population Level

In the past two years, Wayne Memorial Community Health Centers (WMCHC) earned recognition from Million Hearts for achieving greater than 80% hypertension control as an organization and in 6 individual primary care sites. Join PACHC and presenters from WMCHC for a webinar on May 21, 2025, from 12:00 – 1:00 pm to learn how staff leveraged health information technology, evidence-based practice, and team-based care to establish hypertension control.
