Nominations are due July 31, and awards will be presented at the PACHC Annual Conference in October. Patient Choice Award posters and nomination forms were recently mailed to your main health center location or administrative office. Encourage patient participation by displaying these materials prominently. Please note that some English-language Patient Choice nomination forms were distributed with a printing error; an email sent to CEOs outlines the issue and provides instructions and replacement forms to ensure nominations are completed correctly.
